I would guess maybe a terminal came off the inside banana plug connector inside the transformer. unless you know what you are doing I would not suggest you take it apart yourself the internals can easily be damaged if you don't know what your doing and the circuit boards can be quite expensive and mth will not sell them to you anyway.

the one thing I would suggest is check to see if your circuit breaker is tripped and if tripped reset it ok!

p.s. the circuit breaker are behind the left and right back side of the transformer behind the control handles ok

PHILG - Not to worry.  Your Z 4000 is not giving up the ghost.  The same thing happened to my Z4000 and I've been running trains like this ( with out the 14 volt constant output ) for at least a year.   I figured its just like Alan said.  

The Z runs my trains fine.  I use my Lionel ZW to power accessories.  I will take the Z 4000 transformer over to MTH ( they are 10 minutes from my house ) one day for repair and use my second  ZW to power the trains.   MTH repair turn around time is next to forever so that it is why I haven't taken it to them sooner. 

I think you are fine to run keep running your trains with your Z 4000 ... not unless you need that 14 volt constant output to power accessories.
