One other thought: cover the main stack with your finger and blow into the whistle steam opening. Sometimes an air bubble will block the smoke tube.

Thanks @Rider Sandman for the hint! That worked, a little extra smoke fluid, let the engine warm up a bit then I plugged the smoke stack and blew down the whistle steam hole.
