Are you making sure that you're throwing back the Run/Program switch and not the bluetooth switch? The run program switch is on the engineer side of the locomotive, a couple of inches behind the cab, and is the one closest to the edge of the locomotive.
Also, for Lionel steam engines it's normal for the front headlight to remain on when the engine is switched to reverse

Just re-read the manual... and said a lot of not-nice words. Flipped the -correct- switches and now it's running perfectly fine. I need a picard level facepalm for this mistake. (Don't try setting up new engines right after a long day of work)
