Since there doesn't seem to be any new posts on this locomotive, I'll continue on this thread.
My second B&O A5 goes back to the dealer tomorrow. Both lost sound. I checked the tether carefully each time, and it was fully seated. The tether connection, by the way, is in my opinion, the one weak spot. it is small and hard to grasp and fit. the locomotive is still under warrenty.
There are lots of positives in this locomotive. iI looks good, smokes voluminously, and has incredible slow-speed operation. I was using it yesterday, both on a short work train, and as a back-and-forth switcher moving a gondola into position for the magnetic crane. Several visitors commented positively on the slow speed.
Whistle is just a "peep," but OK for me for yard work. Bell is great.

If the sound reliability can be improved (n of 2 in my experience), and some kind of tether improvement made, it will be a winner. Has anyone else had sound issues?
