What about the Engineer who does everything correctly, but there is a resulting mishap thanks to a poor, or dopey driver.

The Engineer has to go through all sorts of questioning and an investigation and GOD forbid if the worse occurs of a fatality and/or injury.

The Engineer has to live with replaying the event in his mind and dreams, for the rest of time.
