Well,

I discovered the screws holding the receiver for the tether were not allowing the rear drivers to seat tight against the frame. This caused the rear drivers to sit at an angle. As the engine rode around curves or switches the rear drivers would lift up and jump the rails. 

It appears this problem is solved
