I have a A/Flyer 18B transformer, when I checked the variable output on either side it's higher 19.7 than my manuals say. Is this normal?

It should be 18V. Assuming the meter is accurate try measuring the voltage with a small load, around 1A, and see if it reads closer to 18V. The 15B, 16B and 19B manuals show 16V. The 18B and 30B show 18V. It is possible Gilbert designed this intentionally with the transformers that had two throttles with one core to allow for the voltage drop when two trains were running. Does the fixed voltage terminal also read 19.7V?
I have a number of these transformers and none put out exactly what they are supposed to. If it shows no sign of damage enjoy the extra voltage.
What is the expected input voltage as stated on the data plate?
What voltage do you measure at the wall receptacle?

Bill: I agree with you completely. The stated output voltage is hardly ever exact.
Arthur: 1) Input voltage would indeed effect output voltage and 2) what are smoke pellets????
