Ok, so what I'm doing is designing a layout with an inside O-36 loop and an outside O-45 loop in Anyrail 6. The spacing between center lines on these loops comes out to 4.5 inches. None of the Atlas turnouts come out to this center spacing when put together as a crossover. Has anybody put a crossover between these two? The closest I can come is 2 O-54 turnouts, and I'll have to cut a custom piece about 1 inch long to come up with a correct center spacing. Is that the best I can do, or does anyone else have another idea?

The O54's are a good fit at the 4.5" C to C track spacing.  Eventually you will need to do some custom fitting. Atlas has track end pieces that can be used to alter standard pieces as needed.  Similar to lower left in this picture.  The track can be cut carefully with a 32 tooth per inch hacksaw or a Dremel tool with a cut off blade, which requires eye protection.  I eventually tweaked the ends with a bench disk sander.  The Atlas plastic end pieces, (6057), will slide on the  ends of the cut track that are cut to length. 

Some custom cuts here.

At one time there were O54 shorts and O54 longs, which had an additional piece.  These two O54's in this configuration required a piece between them . Curves, lower right,  were also cut to match the layout area.

Note:   There is track isolation on this cross over, above.  Track near the wall is a different circuit from the right hand track.

 

One of the smallest custom cuts is bottom center of this picture, Three ties, to accommodate a small change in direction.  Have fun with your layout design.   Tough fit with the two track crosses and an O36 switch between.

With the standard drop in switch track 036/045/054 you are correct the closest you will get would be using the 054 turnouts. I made a quick design using the pieces you would need to get the best fit without making custom cuts. There is a mix of smaller standard straight pieces in the design, 6015, 6052, 6053. Although there is a slight misalignment in several joints when you connect it all together it will not be noticeable.
