Mary Grace: The "knob" I think you're referring to would be the brake wheel. As Jim 1939 alluded to above, it would face however the particular train was assembled. Happy railroading-one less thing to worry about!
No shouting. It is the brake wheel I think and is only on one end ("B end"). All cars have them someplace and the RRs did not worry about which they faced. In other words they did not turn them around to make it face one way or another.
In reality the manual Brake Wheel on Box Cars travels in what ever direction the Box Car happens to be sitting, when the Yardman directs it's hook up, with the main Train.
The brake wheel direction doesn't matter. I prefer them facing rear just to make it look more interesting after a train passes by, but I really don't give it any effort just like the real railroads.
