Watched your video, and hate to break it to you, but both signals show the center light out when lit to "Clear."  At least, it looks that way to me.  Try "reseating" both signals and see if that fixes problem.  The fact that it lights in "Stop" and "Proceed with Caution" modes means the LED is still good, so if "reseating" doesn't fix the issue, you most likely have some kind of interior electrical issue.

Hi all, thanks for the replies, I tried reseating the signal heads, to no avail, same issue. Yes, the center led's work, as in not burned out, but just aren't getting power in the "clear" aspect. I think the problem may be the led board in the signal head itself. I'm not sure how to go about fixing that short of just buying replacement signal heads?

Umm, each side of the bridge has a circuit board in the base.  Try the signals in the opposite direction first and see how they work.  The circuit boards in each leg of the bridge controls the signals for one direction.  All the controlling happens on those two circuit boards.  The light heads are, well, just the lights. If the center light illuminates sometimes, then the problem is not in the signal head nor the wiring.

@Jon G posted:

The sockets in those signal bridges are fragile and the socket receptacles can spread causing them to not grip the pin on the signal head.  You can try to carefully remove the socket and tighten the connector that grips the pins.

Thanks for the suggestion, I'm going to inspect the socket receptacles and, carefully try tightening the connectors a bit. Also,  got a signal head that works as it should, all the time. In the videos, I switched it with the other one, first, good one on the right side, worked fine. Then tried reversing them with the good one on the left side, it worked. This is why I thought the problem lies with the signal heads themselves.
