All:

I had a headlamp from Harbor Freight (Quantum was the brand) that lasted awhile and then just became flaky (loose or too small wires that have broken).  It was awesome when it worked, but what a PITA.  I am disgusted with it.  

Any suggestions for a replacement?  I am looking for a high quality one.

I bought one of these last summer for use at work when doing my evening locomotive inspections and other locomotive work and like it a lot.  The one I have is rechargeable with a cellphone type USB cable but it appears they also make a regular battery one as well.  I usually charge it during the day by just plugging it into the computer for awhile so it's always ready when I need it in the evening.  That being said, I'm not sure exactly how long the rechargeable battery actually lasts, but it's always good enough for what I've needed it for as long as I keep on top of charging it.  Allegedly it'll get you 4 hours of light, but that's likely on the dimmest settings (which have often been plenty of light for me).

I have Energizer brand headlamps. I have one for around the house and I keep one in my bag at work. I have had mine for a few years. They have been dropped, bumped, thrown and the one I use for work made a trip through the washer and dryer and still works like new. You can get them at any big box retailer for between $15 and $20 depending on model.

IMG_1173IMG_1804 2I got one from Amazon, it is called GRDE, 3181F, it has 3 light settings, bright, work and strobe.  This sucker is bright and I do mean bright when its on high, I can shine it from one end of my attic to the other and see exactly what i'm shining it on and very clearly.  Not bragging here but I have a very large attic and it really works great.  I believe that it is rated at over 1000 lumens.  It comes with a car charger, and a 110V charger.   The batteries are 2 rechargeable 18650's and will last up to 5 hours on low and 4 hours on high.  It is hard anodized aluminum and comes in different colors.  The battery pack is in the back and the whole thing is very comfortable.  The head light is also positionable to help with different working conditions.  It is great for close-up work too.  I paid $50 for it and it has been worth every penny.

Those can be bought online, Amazon lists them.

I have a few small Cyclops lights I liked except for the screwy battery size/type  (they do have AA,AAA, etc too). At least 15 years old now, green and black plastic hasn't gotten brittle.  Likely just found those in a sporting goods isle. (things will more likely be UV and atmosphere stable bought in those isles, and of name brands. Solvent stable is automotive

LED Headlamp with strap. Most sporting goods stores would have several offers.  The one I have uses 3 triple A batteries. is adjustable up and down, has a wide angle beam, and smaller focus beam.  A good bit of Southwestern US hiking is done early morning before the sun is up, with headlamps.  They make enough light, that breaking camp early often disturbs those, who prefer the later start.
