A complete power cycle and factory reset would be my first try.  If that doesn't do it, it may have had a tach failure.  Could be simple, that can happen if the tach reader does bad, a wire breaks from the tach reader, or the tape falls off the flywheel.  It can also be a board.  Since that is the older 5V PS/2, you should hope it's not the board.
