Here is a video from ABC news about a train hitting a tractor trailer yesterday in New York City.  No one was seriously hurt.

The video appears to show that the gates did not come down nor did the lights start to flash until after the train hit the truck.  Do gates malfunction often?

To analyze this one, we need more facts.  Was this on a main track or on an industrial spur?  If the speed limit actually was 15 MPH, was there a special instruction to require that trains or engines approach the crossing prepared to stop until active warning devices are fully operational?  It does seem like the crew moved across the crossing expecting the gates to be down.
