I have a new O48 turnout.  I thought it was working right (only used a few times so I may be mistaken).  I noticed tonight it only switches into the straight position.  

Manually turning it to "turn" activates the red lights.  Grounding the track (anti-derail) will switch it to from "turn" to "straight".  The switch will turn it from "turn" to "straight".  But the switch, nor the anti-derail grounding, will switch it to "turn".

I opened it up.  Have no idea what I'm looking for - but nothing jumped out at me.

Anyone have any suggestions for what to look for?

If you look at the black pie shaped piece at the end of travel there is a microswitch it has to close.

Turn the switch manually to the end of travel and listen for the click of the microswitch. If it's not clicking the turnout doesn't "know" it's thrown in the other direction. It doesn't change the lights and it won't go back.

I used a small piece of electrical tape to act as a shim on the edge of the pie shaped piece so it will close the microswitch. Others have built up the edge with a little CA adhesive.
