Your relay is likely shot and/or just lousy. I had a "just lousy" one I re-engineered and got the horn working every time, even at low throttle setting. If you don't want to fool with that, I believe you can get a replacement relay from Jeff at The Train Tender for a lot less cost and bother than rewiring your whole rig with ERR boards. His e-mail is jeff@ttender.com. He's got just about any replacement part for PW Lionel you might need.

Adjusting the tension screw on the "horn" (buzzer, really) is the other thing I should have suggested and it seems you have done so successfully. I'm glad you have it working consistently now. Many say the horn and motor sounds are part of the "charm" of the PW F3 "growlers." Installing new ERR boards may also adversely affect the value of the set, depending on its condition.
