Is there a decent guide you know of besides the folded piece of paper it came with?

What else do you want to know? I have owned the HO one for years and found the paper that came with it to fully explain everything that the HO one could do.
I'm not sure what you are looking for, but as you know this gauge is primarily useful for 2 rail O scale.

I have the N Gauge one and it is pretty simple, but this one seems to indicate that all my wheel gauges are too narrow. How is that possible? That is not the case with my N Gauge one

3-rail wheels have a wider profile, bigger flanges and the gauge isn't designed to work on tubular track.

For wheel gauge, yes! As previously stated, that NMRA O Scale gauge is for 2-Rail SCALE modeling.
