I have 24 Lionel Command Control switches on my layout, and 22 of them work just fine (20 O36, 4 O48). The other two don't appear to be receiving the command signal (even though one of them is right next to a power feed track). I've observed that if I hold a piece of ground wire (that has the command signal on it) an inch or so over the switch, or if I touch the wire to the outside rail on the switch, it works perfectly. I've tried feeding a ground signal back to the switch through the ground connection used for the switch controller, but with no luck (black wire). Interestingly, the two switches that don't work are both O48.

 

Are these just symptoms of a bad switch? Is there any way to disassemble and fix the issue without having to send the switch back to the store for a replacement?

No, and as I mentioned, I have a power feed (with a signal injection point) on the next track over on one of the switches. I think it was the outer rail grounds on the switches weren't making good contact. In addition to the switches working when I held a signal wire over them, I could also get them working if I pressed down on certain places on the plastic track roadbed.
