Thinking bout buying a used MTH O scale locomotive. It’s supposedly in great condition and has 51 scale miles on it, total run time. Is that considered extremely low miles/run time?

Yes, extremely low- barely test run. Remember that's scale miles. If you go 45 scale MPH for 1 hour, that's 45 miles on the odometer.
Our typical club hours open to the public is 4 hours a night, so easily we put 200+ miles on an engine in one running session- possibly a lot more when running faster passenger trains at scale speed (Freight 45, Passenger 55-70+).
Unless it's thousands of hours, I'd also ignore the clock time, that can run up a lot if you just sit on a powered track.
